6th Grade Required Vaccines
07/25/2019
Tdap and Meningococcal vaccinations are required. HPV is recommended at this time. If your child is age 11 and entering sixth-grade in September 2019, he/she is required by the State of New Jersey to have a Tdap and Meningococcal vaccine. Please make an appointment with your physician as soon as possible to assure compliance with these state requirements before school opens in September. IMPORTANT: There is NO GRACE PERIOD; your child will not be permitted to attend school without these mandated immunizations. Once your child has completed these requirements, please forward the documentation from your child’s physician to the office or our school nurse, Mrs. Sienkiewicz, before the start of school, which is on September 6, 2019, to update your child’s health record.
